Proof of insurance must name the Department of Housing and Community Development as an insured. - Sample Escrow Documents (Microsoft Word)

- Request for Disbursement Form (Microsoft Excel)
Requests for Disbursements should be submitted with invoices or other detailed supporting documentation specified by the Department for each warrant requested by an organization. Faxed copies of Request for Disbursement are acceptable, when followed-up with the original executed document. When your organization is requesting funding for construction and/or rehabilitation activities, remember to deduct the 10% retention from each progress payment, if your contractor has not already done so in the project invoice.
