This is a "ready to print" application label template to be used by the applicant when putting the application binder together and is designed to be used with Avery Label #5167 form. These forms are accessible through MS Word program under Tools/Letters and Mailings/Envelopes and Labels/ Labels/ Options. Then scroll down until you see 5167 Return Address. Select and press OK, then hit New Document. The form should have four columns going across. Place your Avery Label Sheet in the Printer and hit print. You can also see a list of Avery Forms at www.avery.com.
