Notice of Sale or Transfer (PDF)HCD RT 476.8Optional form. This form is not a release of liability although the previous owners of the unit are signing off their ownership of the unit. A full and complete transfer application with signatures from buyers and sellers of a unit still needs to be submitted to HCD in order for ownership to be changed on title. The buyer listed on form HCD RT 476.8 will only be placed as a “Pending Owner” in HCD’s system until a full, complete transfer application is received.
Statement of Facts Salvage Units (PDF)HCD 486.4Required when reporting a unit as uninhabitable, salvaged, or destroyed. HCD Certificate of Title and unit registration card must be submitted with form HCD 486.4.
